Alexander made his acting debut on Netflix's The OA, as Buck Vu. Buck, like Alexander, is a Vietnamese-American transgender teen whose experiences are modeled in part on Alexander's own.
They were cast after an open casting call went viral on Tumblr. Later, they were cast as Vic, a trans kid who the spirit "A" occupies for a day, in the 2018 feature film Every Day, based on the book by David Levithan.
Alexander joined the cast of The Last of Us Part II, the sequel to Naughty Dog's popular video game, in October 2017. They portrayed Lev, a game character who is transgender.
In June 2019, Queerty named them one of their Pride50: "trailblazing individuals who actively assure society remains progressing towards equality, acceptance, and dignity for all gay people" to commemorate the 50th anniversary of the Stonewall Riots, which sparked the contemporary LGBTQ rights movement.
Alexander was also featured on the cover of their 2019 Pride issue, which featured "three rising gay artists whose work and lives are forging new ground for LGBTQ+ visibility." Alexander will star in the independent film Daughter, which will be released in March 2020.
Alexander joined the cast of Star Trek: Discovery in September 2020, playing the first transgender character ever performed by a transgender actor in the Star Trek canon.
